It's the AFK Tavern in Everett, WA. I don't know if they still have this particular drink menu, but their current one (on their site) is still pretty great. It's a gamer bar, so everything is epically nerdy.
I want to go to there.

I'm sure they do, but it takes like 30 minutes to make, then the bartender drinks it themselves.
